OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck by hazard from the damaged overhead crane hooks.    The hauling hooks on the overhead cranes were missing and/or had damaged safety latches designed to prevents equipment/steel beam from coming off the hook while in the midst of hauling the steel beam to areas of the shop, on  or about 11/06/13.         a.  Three-Ton Overhead Crane (#87269) - Safety latch was missing from the hook        b.  Three-Ton Overhead Crane (#87266) - Safety latch spring pin is broken making the safety latch ineffective, loose and not properly secured.  There was a large gap/space where material can slide off  the hook.      c.  Ten-Ton Overhead Crane (#87276) - Safety latch spring pin is broken making the safety latch ineffective, loose and not properly secured.  There was a large gap/space where material can slide off the hook.    Among other methods, one feasible and acceptable method to correct this hazard is to follow the Hooks ASME B30.10-1993, Section 10-1.2.3 Maintenance.

29 CFR 1910.147(c)(4)(ii)(C): The energy control procedure did not clearly and specifically outline the steps for placement, removal and transfer of lockout devices or tagout devices and the responsibility for them:     a.  405 N. Clarendon Avenue, Scottdale, GA (Shop Area) - Employees were exposed to sudden energization or start-up of multi-energy source.  The company's lockout/tagout machine specific procedures did not include specific steps for the removal of locks and restoring the lockout/tagout equipment back into services.  Equipment includes but is not limited to, the Challenge Burn Table, Peddingghaus Anglemaster and Overhead Cranes, on or about 11/06/13.
