Clarendon Ave, Scottdale, GA: Where employee(s) operated a propane powered industrial truck with defects and deficiencies such as but not limited to severely worn tires, broken propane tank slide assembly, large hole in the air filter cover, and broken and missing front and side windshield. The employer failed to remove the powered industrial truck from service until the repairs could be made.

On or about 1/27/2020, at the Wheelabrator equipment in the facility located at 416 N. Clarendon Ave, Scottdale, GA: Employees operated a 15-ton Ace Industries Inc. overhead crane, with a chain sling and dog clamp attachment, to load steel beams in length over 51 feet and weight over 2585 pounds onto a conveyor system. The chain sling and dog clamp attachment did not clear the steel beam while being raised, thereby exposing the employees to struck-by and/or caught-under hazard.
