29 CFR 1926.100(a):   Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:   a)  Worksite, 28 Lincoln Ave. Rockville Centre NY - Employees were not wearing protective helmets and were exposed to being struck by asphalt shingles and tools while walking and working below overhead roofing work; on or about 11/8/13.   Note:  The employer is required to submit abatement certification for this item in accordance with 29 CFR 1903.19.

29 CFR 1926.501(b)(13):  Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employees provided with an alternative fall protection measure undder another provision of paragraph 1926.501(b):   a) Worksite, 28 Lincoln Ave, Rockville Centre NY  - Employees removing existing shingles from a roof approximately 15 feet above the ground, were not protected from falling; on or about 11/8/13.     Note:  The employer is required to submit abatement certification for this item in accordance with 29 CFR 1903.19.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:   a)  Worksite, 28 Lincoln Ave. Rockville Centre NY - Employees were accessing a portion of a flat roof by using an extension ladder that was extended approximately 1ft above the landing; on or about 11/8/13.   Note:   Because abatement of this violation is already documented in the case file, the employer need not submit certification or documentation of abatement for this violation as normally required by CFR 1903.19.
