29 CFR 1926.20(b)(2): The employer failed to initiate and maintain programs that provided for frequent and regular inspections of the job sites, materials, and equipment were made by competent persons designated by the employer:   At the project on or about November 12, 2013, and at times prior thereto, the project had not been inspected or the person that did the inspection was not competent as there were about 25 unguarded rebars and two extension cords with no strain relief.

29 CFR 1926.95(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, were not provided, used, and maintained in a sanitary and reliable condition wherever it was necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact:   At the project on or about November 12, 2013, safety toed shoes were not worn while employees carried and used lumber and circular saws.  Employees wore closed shoes with no toe protection.

29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief was provided which will prevent pull from being directly transmitted to joints or terminal screws:   At the project on or about November 12, 2013, the outside insulation on two extension cords was not connected to the plugs.  This caused pull to be directly transmitted to the plugs joints or terminals.  The extension cords powered a 110 VAC Ridgid circular saw, used to cut lumber.

29 CFR 1926.701(b): Reinforcing steel. All protruding reinforcing steel, onto and into which employees could fall, was not guarded to eliminate the hazard of impalement:   At the project on or about November 12, 2013, and at times prior thereto, about 25 rebars were not guarded to prevent impalement.  The 3/8s inch diameter rebars protruded vertically about 36 to 40 inches from a concrete slab and concrete blocks.

29 CFR 1926.1053(b)(4): Ladders were not used only for the purpose for which they were designed:   At the project on or about November 12, 2013, an 8 foot long step ladder was used in the folded up position to access the roof.  The lowest eve of the roof was about 8 feet above the ground.
