29 CFR 1910.147(c)(4)(ii): Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D)(h): a/ Betar, Inc./Somerset, NJ Site: Production Area. The lockout program provided was not documented to include, but not limited to: 1/ the program was not documented and tailored to the site specific equipment to be worked on 2/ there were no specific steps documented to de-energize the equipment 3/ means to control the use and identity of locking devices 4/ designated personnel involved in the program, etc. Equipment include but not limited to: All hard wired production machines and cord/plug connected machines where the employee does not have exclusive control over the cord/plug (Boring Machines, Lathes, Gun Drilling Machines, Surface Grinders, pedestal/bench mounted drills, etc:) Observed on/or about 11/08/2013

29 CFR 1910.212(a)(1): Machine guarding was not provided to protect operator(s) and other employees from hazard(s) created by the cutting action of saw blade and rotating parts of chucks: a / Betar, Inc./Somerset, NJ Site: Production Area. 1/ Royal M/T 1016JA Horizontal Metal Cutting Saw. The unused portion of saw blade approx. one inch wide, approx. 8 inches long on the left and 3 inches long on the right side of the cutting area (used to cut metal), were not guarded to prevent accidental contact during operation. 2/ All Production Machines with Chucks approx. 6 inches to 24 inches in diameter, including but not limited to Nomlira Boring Machines, Sigma Lathe(s), Gun Drill machines, etc. Guards were not provided to prevent accidental contact during operation. Observed on/or about 11/08/2013
