29 CFR 1910.305(b)(1)(i): Openings through which conductors entered cutout boxes, cabinets, or fittings were not effectively closed:  On or about November 6, 2013, the employer did not ensure that electrical equipment was free from recognized hazards. Employees were exposed to electrical shock hazards in the following instances:     a)    The 240V nominal cable going into the electrical panel attached to the transformer did not have a bushing effectively closing the opening.    b)    Next to the Toolpusher trailer, the 480/240V high voltage transformer top cover was damaged leaving openings into the transformer.      c)    The cover to the electrical panel connected to the transformer would not close due to a live electrical wire caught between the door and panel.    d)    Inside the electrical plant, the electrical control panel doors labeled Water Pump #2 and Camp Transformer were missing bolts and would not close; the openings exposed live parts.
