29 CFR 1910.22(b)(1): Aisles and passageways were not kept clear and in good repair, with no obstruction across or in aisles that could create a hazard: On or about November 6, 2013, passageways were not kept in good repair. Employees were exposed to trip hazards in the following instances: a) On the westside of the rig floor, the stairway landing was not flush the floor; it was sticking up approximately 2-inches. b) In front of the mud pump generator, the metal suitcase used to store electrical wiring had one metal door cover sticking up approximately 3-inches.

29 CFR 1910.305(g)(1)(iv)(C): Flexible cords and cables were used where run through doorways, windows, or similar openings: On or about November 6, 2013, on the backside of the employee trailer, two electrical extension cords powering the sewage pumps were run through the back window. Employees were exposed to electrical shock hazards.

29 CFR 1910.305(b)(1)(i): Openings through which conductors entered cutout boxes, cabinets, or fittings were not effectively closed: On or about November 6, 2013, the employer did not ensure that electrical equipment was free from recognized hazards. Employees were exposed to electrical shock hazards in the following instances: a) The 240V nominal cable going into the electrical panel attached to the transformer did not have a bushing effectively closing the opening. b) Next to the Toolpusher trailer, the 480/240V high voltage transformer top cover was damaged leaving openings into the transformer. c) The cover to the electrical panel connected to the transformer would not close due to a live electrical wire caught between the door and panel. d) Inside the electrical plant, the electrical control panel doors labeled Water Pump #2 and Camp Transformer were missing bolts and would not close; the openings exposed live parts.
