29 CFR 1910.22(b)(1): Aisles and passageways were not kept clear and in good repair, with no obstruction across or in aisles that could create a hazard: On or about November 6, 2013, passageways were not kept in good repair. Employees were exposed to trip hazards in the following instances: a) In front of the mud pump generator, the metal suitcase used to store electrical wiring had one metal door cover sticking up approximately 2-inches at the highest point. b) On the shale shaker side of the rig floor, the stairway landing was not flush the floor; it was sticking up approximately 2-inches.

29 CFR 1910.305(g)(2)(iii): Flexible cords were not connected to devices and fittings so that tension would not be transmitted to joints or terminal screws: On or about November 6, 2013, the outside light positioned above the Toolpusher trailer plugged into the side of trailer had the strain relief busted on the male end. Employees were exposed to electrical shock hazards.
