29 CFR 1910.67(c)(2)(iv): Employees does not always stand firmly on the floor of the basket, and does not sit or climb on the edge of the basket or use planks, ladders, or other devices for a work position. On or about November 13, 2013, employer did not ensure employee stood on the floor of the basket. Employee operating an aerial lift climbed out of the basket on to an adjacent canopy roof. This codition exposed employee fall hazards greater than 10 feet.

29 CFR 1910.67(c)(2)(v): A body belt was not worn and a lanyard attached to the boom or basket when working from an aerial lift: On or about November 13, 2013, employer did not ensure that employee working on an aerial lift was wearing fall protection equipment. This condition exposed employee to fall and hazards greater than 10 feet.
