29 CFR 1910.66(i)(1)(ii)(A): All employees who operate working platforms were not trained in the recognition of, and preventive measures for, the safety hazards associated with their individual work tasks: On or about December 11, 2014, the employer did not provide hazard recognition training for employees performing activities including but not limited to working from an aerial device (boom truck). Employees were working without any form of fall protection and were exposed to a fall hazard greater than 4 feet.

29 CFR 1910.67(c)(2)(v): A body belt shall be worn and a lanyard attached to the boom or basket when working from an aerial lift: On or about December 11, 2014, on the corner of the Panda Express parking lot on Zaragoza Rd., an employee was observed working from an aerial device (boom truck) without any form of fall protection and was exposed to a fall hazard greater than 4 feet.
