29 CFR 1926.403(b)(1): 29 CFR 1926.403(b)(1): Employer did not ensure that electrical equipment was free from recognized hazards that were likely to cause death or serious physical harm to employees:  At this location: Employees were exposed to shock hazards from contact with electrical equipment that contained the following deficiencies:  a) North end of the workplace, ground level: electrical power panelboard used as a workplace temporary electrical service did not contain a means to disconnect all conductors in the building or other structure from the service entrance conductors in violation of Article 230.70 of the National Electrical Code,  b) Throughout the worksite building under construction: Type MC cable used by employees as extension cords that supplied electrical power to portable electrical equipment used by employees engaged in construction operations were attached to 15 ampere, 120 volt, 3 pole male plugs in violation of Article 300.15 of the National Electrical Code,  c) North end of the workplace, ground level: Employees were exposed to shock-hazards from contact with two Type MC cables used as Aerial Cables and transferred electrical power of 120 volts AC from the  top of the ground level temporary power pole to the building under construction; but were not suspended/supported from an overhead messenger cable as required by Article 330.10(B)(4) of the National Electrical Code.

29 CFR 1926.404(b)(1)(ii): On a construction site, where an assured equipment grounding program was not utilized, all 120-volt, single-phase, 15 and 20 ampere receptacle outlets which were not a part of the permanent wiring of the building or structure and which are in use by employees did not have approved ground fault circuit interrupters for personal protection:  At this location: Employees were exposed to shock hazards from contact with portable electrical equipment not protected by ground fault circuit interrupters. The portable electrical equipment used by employees engaged in construction operations and not powered by ground fault circuit interrupters operated at 120 volts AC in the following locations:  a) Ground floor, north end of the worksite, temporary power pole panelboard at a height of 36 inches above the ground,   b) 2nd floor, northeast interior area of the building at a height of 34 inches above the workplace floor, a 15 ampere duplex receptacle.  c) 2nd floor, northeast interior area of the building at a height of 56 inches above the workplace floor, a 15 ampere duplex receptacle, a 15 ampere duplex receptacle.  d) 3rd floor, northeast interior area of the building at a height of 51 inches above the workplace floor, a 15 ampere duplex receptacle.

29 CFR 1926.405(a)(2)(ii)(B): 29 CFR 1926.405(a)(2)(ii)(B): 29 CFR 1926.405(a)(2)(ii)(B): Runs of open conductors were not located where the conductors will not be subject to physical damage. The conductors were not fastened at intervals not exceeding 10 feet (3.05 m); with branch-circuit conductors laid upon the floor.     At this location: Employees were exposed to shock hazards from contact with branch circuit conductors enclosed within type "MC" cable (metal clad) that operated at 120 volts AC, and were laid on the floor in the following locations:  a) 2nd floor of the workplace, the energized MC cable ran in a north/south direction while it was laid upon the floor,  b) 3rd floor of the workplace, the energized MC cable ran in a north/south direction while it was laid upon the floor,  c) 3rd floor of the workplace, the energized MC cable rain in an east/west direction from the 3rd floor building window to      the center of the hallway floor.

The installed duplex receptacles that did not have a cover; operated at 120 volts AC, and were used by employees engaged in construction operations at the following locations:  a) 2nd floor, northeast interior area of the building at a height of 34 inches above the workplace floor,  b) 2nd floor, northeast interior area of the building at a height of 56 inches above the workplace floor,  c) 3rd floor, northeast interior area of the building at a height of 51 inches above the workplace floor.
