29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents: a. On or about November 12, 2013, at the west side of the building, three employees using air nail guns to install roof shingles were not wearing eye protection, exposing themselves to being struck by flying debris or nails. b. On or about November 12, 2013, at the north side of the building, one employee using an air nail gun to install ridge caps was not wearing eye protection, exposing himself to being struck by flying debris or nails.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b): a. On or about November 12, 2013, at the north side of the building, one employee installing ridge caps on a 6:12 pitch residential roof was not protected from a 9 feet 7 inches fall hazard by the use of a fall protection system. b. On or about November 12, 2013, at the west side of the building, three employees installing roof shingles on a 6:12 pitch residential roof were not protected from an 9 feet 7 inches fall hazard by the use of a fall protection system. c. On or about November 12, 2013, at the east side of the building, one employee walking on a 6:12 pitch residential roof picking up waste close to the edge was not protected from a 9 feet 7 inches fall hazard by the use of a fall protection system.
