OSH ACT of 1970 Section (5)(a)(1): The employer does not furnish employment and a place of employment which are free from recognized hazards that are causing or likely to cause death or serious physical harm to employees in that employees are exposed to the hazard of crushing injuries:    The employer does not ensure the lifting devices are marked with the rated load where it is visible and a nameplate of the permanent marking is affixed on the lifting device containing the manufacturer's name and address, serial number, lifter weight and the rated load.  The following hoists which are in violation were most recently observed on November 13, 2013.      Two Harrington Hoists are located in M1, previously known as the North Building.  Two Harrington Hoists are located in M2, previously known as the South Mill, located in the Production Building.  Three Harrington Hoists are located in M3, previously known as the Pellet Mill, located in the Production Building.    To abate this violation the employer must ensure that the lifting devices are properly marked.    Pursuant to 29 CFR 1903.19, the employer must submit documents describing the steps it is taking to ensure compliance, including removal of the unmarked lifting devices and installation and use of appropriately marked lifting devices.    The All-Feed Processing and Packaging, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard Section 5(a)(1), which was contained in OSHA inspection number 314850421, citation number 3, item number 1and was affirmed as a final order on February 11, 2013, with respect to a workplace located at 717 West Division Street, Galva, IL 61434-1636.    The All-Feed Processing and Packaging, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard Section 5(a)(1), which was contained in OSHA inspection number 310802228, citation number 1, item number 2, and was affirmed as a final order on February 3, 2009, with respect to a workplace located at 717 West Division Street, Galva, IL 61434-1636.

29 CFR 1910.178(c)(2)(vii): Power-operated industrial trucks designated as DY, EE, or EX are not used in atmospheres in which combustible dust is not normally in suspension in the air by normal operation of equipment or apparatus but where deposits or accumulations of  such dust could be ignited by arcs or sparks originating in the truck.    Powered industrial trucks are used in areas which are Class II, Division 2 locations where dust is produced by the processing of products including, but not limited to, starch and cellulose fiber.  This violation was most recently observed on November 14, 2013, in the production area, where a Toyota (Model 8FGU25) LPS powered industrial vehicle was being used to move product.    To abate this violation, the employer must ensure that employees only use their electric powered industrial vehicles in the production area.     Pursuant to 29 CFR 1903.19, the employer must submit documents describing the steps it is taking to ensure compliance, including a copy of the site-specific program detailing the procedures used to ensure compliance and training of employees.    The All-Feed Processing and Packaging, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.178(c)(2)(vii), which was contained in OSHA inspection number 314850421, citation number 2, item number 1 and was affirmed as a final order on February 11, 2013, with respect to a workplace located at 717 West Division Street, Galva, Illinois, 61434-1636. 29 CFR 1910.307(c): Equipment, wiring methods, and installations of equipment in hazardous (classified) locations are not intrinsically safe, approved for the hazardous (classified) location, or safe for the hazardous (classified) location:     The employer does not ensure that electrical equipment is intrinsically safe and approved for Class II, Division 2 locations.  This violation was most recently observed on November 13, 2013, in the grinding and processing areas where the following material hoists and production equipment motors are operating:       Two Harrington Hoists are located in M1, previously known as the North Building.  Two Harrington Hoists are located in M2, previously known as the South Mill, located in the Production Building.  Three Harrington Hoists are located in M3, previously known as the Pellet Mill, located in the Production Building.   Baldor industrial motors (including but limited to Cat.'s M3704, M3611T, M3609, and M3546T).    Pursuant to 29 CFR 1903.19, the employer must submit documents describing the steps it is taking to ensure compliance, including removal of unapproved electrical equipment and installation of appropriate electrical equipment as needed.     The All-Feed Processsing and Packaging, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.307(c), which was contained in OSHA inspection number 314850421, citation number 2, item number 2 and was affirmed as a final order on February 11, 2013, with respect to a workplace located at 717 West Division Street, Galva, Illinois 61434-1636.
