29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: On or about November 14, 2013 and times prior thereto, the chip conveyor feeding the silo had unguarded pinch points between the moving belt and V rollers exposing employees to caught in hazards.

29 CFR 1910.261(k)(1): Paper machines were not equipped with devices that would stop the machine quickly in an emergency. The devices did not consist of push buttons for electric motive power (or electrically operated engine stops), pull cords connected directly to the prime mover, control clutches, or other devices, interlocked with adequate braking action. The devices were not tested periodically by making use of them when stopping the machine and were not so located that any person working on the machine could quickly disconnect the machine from the source of power in case of emergency. On or about September 7, 2013 and times prior thereto, the re-reeler on the #1 paper machine was not equipped with an adequate braking mechanism. In the event of an emergency stop activation the roller would continue its rotation and expose employees to the hazards of being caught by the moving roller.
