29 CFR 1910.151(c) Where the eyes or body of any person may be exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use.  In the laundry for employees who used corrosive chemicals including but not limited to Solid Stain-A-Way, Stain Blaster R, and Stain Blaster B, an approved eyewash was not available for immediate emergency use.

29 CFR 1910.215(b)(9)  Where the operator stands in front of the opening, a peripheral protecting  member or tongue guard, was not in place to adjust to the constantly decreasing diameter of the wheel. The distance of the opening exceeded one-fourth inch:  The Dayton 6" bench grinder used approximately once per month did not have tongue guards on both sides to adjust to the constantly decreasing diameter of the wheels.  The distance of the opening between the wheel periphery and the grinder measured 1/2" on both sides.

29 CFR 1910.303(g)(1)(i)(A)  The depth of the working space in the direction of access to live parts was less than indicated in Table S-1. Distances shall be measured from the live parts if they are exposed or from the enclosure front or opening if they are enclosed:  a) Kitchen, electrical panel box servicing toaster, lights, Robocoupe, microwave, blender, and blixer, etc. (208/120V) were blocked by serving tray stands and a table approximately 16 inches from the electrical panel box.   b) Kitchen, the knife switch shut-off box below the dish draining tables was blocked by racks.    c) Division 700 soiled utility room, the electrical panel boxes were blocked by biohazard barrels.
