29 CFR 1910.157(c)(1): The employer shall provide portable fire extinguishers and shall mount, locate and identify them so that they are readily accessible to employees without subjecting the employees to possible injury. 1. In the middle of the production area, there was a fire extinguisher situated on the floor near the sink that was exposed to physical damage. 2. In the middle of the production area, there was a fire extinguisher located underneath the sink on the floor exposed to physical damage.

29 CFR 1910.178(l)(1)(i): The employer shall ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l). 1. At the time of the inspection, three (3) employees had been operating a fork truck (Hydroelectric Lift Truck) without training and/or certification to drive a fork truck.
