29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:  JOB SITE: 20 Millpark Court, Maryland Heights, MO 63043, on or about 7/21/2017, the company failed to established a program consisting of energy control procedures, employee training and periodic inspections for but not limited to the Polar Mohr Shears, Model: 92X, S/N: 7411062 and Datien, Micro Computer Cutting Machine, Model: CT-11, S/N:0050106.  Workers were exposed to an electrical hazard.

29 CFR 1910.178(l)(4)(iii): An evaluation of each powered industrial truck operator's performance shall be conducted at least once every three years.  JOB SITE: 20 Millpark Court, Maryland Heights, MO 63043, on or about 7/21, 2017, the employer failed to conduct an evaluation of the powered industrial operator's performance at once every three years.  Their evaluation was conduct in December 2013.
