29 CFR 1926.502(d)(21): Personal fall arrest systems were not inspected prior to each use for wear, damage, and other deterioration, and/or defective components were not removed from service: (a) New ICU wing at WellStar Medical Center, 8820 Hospital Drive, Douglasville, GA: Employees were engaged in steel erection activities (installing columns for a 4-foot parapet wall) on the deck of a multi-story building and the employer did not ensure that PPE (harnesses, retractable lifelines and chokers) were not removed from service.
