Align Roofing Company, LLC, formally known as, Align Roofing, LLC was previously cited for a violation of this Occupational Safety and Health Standard, or its equivalent standard, 29 CFR 1926.501(b)(13), which was contained in OSHA Inspection number 315482133, Citation number 01, Item number 001, issued on December 12, 2011, with respect to a workplace at 923 1st Avenue S., Jacksonville Beach , Florida 32250. The final order for this citation was July 18, 2012. 29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, a personal fall arrest system, or an alternative fall protection measure as provided by another provision of paragraph (b) of this section: a. On or about November 13, 2013, on a 4:12 pitch roof, two employees were exposed to a 10-foot, 6-inch fall hazard in that they were not protected by a fall protection system.
