29 CFR 1926.501(b)(4)(ii): Each employee on a walking/working surface was not protected from tripping in or stepping into or through holes (including skylights) by covers:    a) On or about December 17, 2016, the employer failed to ensure that the employees were protected from falling through skylights by installing covers. An employee fell more than 20' through a skylight onto the concrete floor below when he stepped into an unprotected skylight resulting in multiple fractures.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest system, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system:     a) On or about December 17, 2016, the employer failed to ensure that the employees were protected from falling more than 20 feet onto a concrete floor while conducting roofing work on a 4:12 pitch roof, exposing the employees to falls that could result in serious injures or death.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:    a) On or about December 17, 2016, the employer failed to ensure that all the employees assigned roofing duties were trained on the recognition and prevention of fall hazards when the employees were exposed to falls of more than 20 feet onto the concrete floor, exposing employees to serious injures or death.
