29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, a personal fall arrest system, or an alternative fall protection measure as provided by another provision of paragraph (b) of this section: a. On or about November 13, 2013, the second story window opening did not have a guardrail, exposing employees to a 13-foot fall hazard. b. On or about November 13, 2013, on a 4:12 pitch roof, an employee performing house wrap insulation work was not protected from a 9-foot, 6-inch fall hazard by the use of a fall protection system.
