29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:  a. On or about October 2, 2014, on the roof, an employee using a Ridgid pneumatic air stapler while installing vinyl components was not wearing safety glasses, exposing him to eye injuries.  b. On or about October 2, 2014, on the ground floor, an employee was using a Ridgid skill saw while cutting vinyl components was not wearing safety glasses, exposing him to eye injuries.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  a. On or about October 2, 2014, on the 7:12 pitch roof, an employee was installing vinyl along the soffit of the residential house without a fall protection system, exposing him to an 9 feet fall hazard.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a.  On or about October 2, 2014, the extension ladder used to access the roof did not extend 3 feet above the eave, exposing employees to a 9 feet fall hazard.

29 CFR 1926.1053(b)(4): Ladder(s) were used for purposes other than the purpose for which they were designed:  a.  On or about October 2, 2014, an employee on the roof, was using two 8-foot aluminum Louisiana stepladder with a plank stationed on the top rungs in order to install vinyl along the soffit of a residential house, exposing him to a 7 feet 8-inch fall hazard.
