29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, struture, personnel hoist, or similar surface was not used. Cross braces were used as a means of access. This violation was discovered on 11/18/2013 where adequate access to the roof of building was not provided to employee exposing the worker to fall related injuries.

29 CFR 1926.502(d)(16)(ii): The personal fall arrest system did not limit the maximum arresting force on an employee to 1,800 pounds (8 kN) when used with a body harness This violation was discovered on 11/18/2013 where the lanyard attached to an anchorage and connected to the body harness did not have a deceleration device or shock absorbing component to protect employee in the event of an accidental fall.
