On or about November 21, 2013, Parts Welding Department, Exit "C": The identified emergency exit door was provided with an automatic barrel lock, which actuated when the door was closed. In case of an emergency in the Parts Welding Department, access to the outdoor open space would be prevented.

29 CFR 1910.36(g)(2): Exit access(es) were not at least 28 inches (71.1 cm) wide at all points: A. On or about November 21, 2013, Parts Welding Department, Exit "C": Access to the idenfied emergency exit was reduced to below 28 inches by the placement of a pallet in the aisleway, a rolling metal cart and an uprighted pallet placed to keep leaves from blowing into the shop. In case of an emergency, employees would not have direct access to the emergency exit.

On or about November 21, 2013, Sheet Metal Department: The workspace in front of a 240V electrical panel with a three-phase ground leg was used for storage. The panel door could not open fully due to the placement of a metal rack. The panel was identified as powering a shear, a brake, an alpha punch and a surface grinder.
