29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  a) Job Site - Employees were not provided fall protection training as evidenced by employees on a flat roof installing tar paper and not using fall protection and exposed to a 30 ft, fall hazard, on or about 11/18/2013.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1926.1060(a): The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would enable each employee to recognize hazards related to ladders and stairways and train each employee in the procedures to be followed to minimize these hazards:  a) Job Site -  Employees were not provided ladder training as evidenced by employees climbing a portable metal ladder that did not extend 3 ft, beyond the upper landing and exposed to a 33 ft, fall hazard, on or about 11/18/2013.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

Capitol High Services, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 907916, citation number 1, item number 1, and was affirmed as a final order on 07/01/2013, with respect to a workplace located at 939 M Street, NW, Washington DC, 20001.

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladders length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist emmployees in mounting and dismounting the ladder:  a) Roof - Employees were observed climbing a portable metal ladder, 33 ft above the ground,  where the top of the ladder extended 24 inches above the upper landing, on or about 11/18/2013.  Capitol High Services, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.1053(b)(1), which was contained in OSHA inspection number 580618, citation number 1, item number 2b, and was affirmed as a final order on 10/05/2012, with respect to a workplace located at 112 5th Street, SE Washington DC 20007.
