29 CFR 1910.178(q)(7): Industrial trucks were not examined before being placed in service: Building 83, Logistics, employees were operating a Taylor-Dunn Electric Cart and the employer did not ensure that powered industrial trucks were examined before being placed in service. The Veterans Health Administration was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.178(q)(7), which was contained in OSHA inspection number 531579, citation number 1, item number 2b and was affirmed as a final order on February 11, 2013, with respect to a workplace located at Battle Creek VA Medical Center, Battle Creek, MI.

29 CFR 1910.178(l)(2)(ii): The employer did not ensure that each operator had successfully completed the training consisting of a combination of formal instruction (e.g., lecture, discussion, interactive computer learning, video tape, written material), practical training (demonstrations performed by the trainer and practical exercises performed by the trainee), and evaluation of the operator's performance in the workplace. Building 83, Logistics, employee required to operate the Taylor-Dunn electric cart was not provided training on the safe operation of the cart that consisted of practical training, and an evaluation of the operators performance in the workplace.
