29 CFR 1910.23(d)(1)(iv) as required by 1960.8(b): On stairways more than 44 inches wide but less than 88 inches wide, one handrail on each enclosed side and one stair railing on each open side.  Building 27, the stairway with 6 risers leading up to the electrical shop was not provided with a handrail on the enclosed side there by exposing employees to a fall hazard.

29 CFR 1910.24(h) as required by 1960.8(b): Standard railings were not provided on the open sides of all exposed stairways and stair platforms:  Building 27, the stairway with 6 risers leading up to the electrical shop was not provided with a standard railing in that the railing did not have a midrail, there by exposing employees to a fall hazard.

In building 26, in the tile storage area the exit leading out of the building was not marked by a sign reading "Exit".  b. In building 26, in the vehicle maintenance area break room, the exit leading out of the building was not marked by a sign reading "Exit".  c. In building 27, in the mechanical shop  the exit leading out of the building was not marked by a sign reading "Exit".  d. In building 30, in the grounds storage room, the exit leading out of the building was not marked by a sign reading "Exit".  e. In building 145, in the warehouse,  the southwest exit leading out of the building was not marked by a sign reading "Exit".

29 CFR 1910.23(c)(1)as required by 1960.8(b): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toeboards:   Building 26, Tile Storage Area in the mezzanine , an open-sided platform did not have an adequate guardrail in that, it did not have a midrail there by exposing employees to a fall hazard of up to eight feet in height.     The Veterans Health Administration was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.23(c)(1), which was contained in OSHA inspection number 588298, notice number 1, item number 1 and was affirmed as a final order on July 8, 2013, with respect to a workplace located at 4646 John R. St., Detroit, Michigan 48201.
