29 CFR 1910.1200(f)(5): The employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked:    (a) Terre Hill Concrete Products - The employer did not ensure that each group of concrete blocks that were palletized were tagged with a sign identifying the blocks as containing crystalline silica, on or about November 20, 2013.      Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1910.1200(h)(1): The employer did not provide employees with effective information and training on hazardous chemicals in their work area at the time of their initial assignment, and whenever a new physical or health hazard the employees have not previously been trained about was introduced into their work area:     (a) Terre Hill Concrete Products  - The employer did not provide information and training on crystalline silica to employees who are exposed to silica while manufacturing concrete blocks, on or about November 20, 2013.     Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1910.95(g)(8)(ii)(B): The employer did not ensure that employees already using hearing protectors were refitted and retrained in the use of hearing protectors and provided with hearing protectors offering greater attenuation when a standard threshold shift had occured:  (a) Terre Hill Concrete Products - The employer did not refit an employee with hearing protection with greater attenuation when the audiometric test showed that a standard threshold shift occurred on November 13, 2013. The employee wore foam earplugs, on or about November 20, 2013.   Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1910.134(c)(1)(i):  Whenever respirators were required by the employer, the employer did not establish and implement a written respiratory protection program with worksite-specific procedures. The employer did not include in the program information on procedures for selecting respirators for use in the workplace:  (a) Terre Hill Concrete Products - The employer did not include in the written respiratory protection program provisions on the procedures for selecting respirators that were being used in the workplace. Employees are required to wear 3M particulate respirators 8210, N95 while grinding, and cleaning the mixing room, on or about November 20, 2013.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
