29 CFR 1910.23(a)(8)(ii): Every floor hole into which persons can accidentally walk were not guarded by a floor hole cover of standard strength and construction:    a. On or about 11/20/2013, at the front of the plant - employees were exposed to a fall hazard because the cover of a large rounded drain was not installed.      b. On or about 11/20/2013, at the back of the plant - employees were exposed to a fall hazard because the cover of a large rounded drain was not installed.      c. On or about 11/20/2013, at the back of the plant - employees were exposed to a fall hazard because the cover of a large square drain was not installed.      d. On or about 11/20/2013, at the plant - the floor in the production area had many holes and indentations exposing the employees to a trip and fall hazard.

On or about 11/20/2013, glass crushing machine - employees working from the machine platform were exposed to an 8 feet 5 inches fall hazard, in that, the chains used as guard rails were not connected.       b. On or about 11/20/2013, PET baler machine - employees working from the machine platform were exposed to a 5 feet 5 inches fall hazard because the platform was missing guard rails.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    a. On or about 11/20/2013, at the PET baler - the electronic cut-off switches were removed and by-passed in a manner which allowed the machine to continue operating at all times. All the switches were placed in a milk crate on top of the automatic wire tie section of the baler.
