The employer did not ensure that employee coordination was in place and adequate communication was established.     2. The employer did not ensure that non-essential employees were clear of active high pressure lines during lubricator operations by a third-party operator.     On November 20, 2013  at FM 2688 Espejo Ranch Gate #010CHKBTRIC2H an employee was fatally injured while working near high-pressure lines.  Employees  were exposed to struck-by hazards from approximately 4000 pound per square inch pressure in the lines.
