29 CFR 1910.134(d)(1)(iii): The employer did not identify and evaluate the respiratory hazard in the workplace:  (a) Calfrac Well Services Corporation, at Well Pad SG 714-44, Parachute, CO: On and preceding 7/29/15, Calfrac Well Services Corporation did not identify and evaluate the respiratory hazard in the workplace.  Employees engaged in hydraulic fracturing of oil and gas well used a gravity fed silo system for pushing fracking sand.  The fracking sand consisted of either Preferred Pipeline Frac Sand Proppant 100 mesh (containing 40% crystalline quartz) or Imerys ProLite Proppant 40/70 mesh (containing 1% crystalline quartz).  The transport and blending of the hydraulic fracturing materials released respirable silica and respirable dust.  Employee exposure to respirable silica or respirable dust had not been determined for the gravity fed silo system.  This condition potentially exposed employees to a respiratory hazard.

Employees engaged in hydraulic fracturing of oil and gas well used a gravity fed silo system for pushing fracking sand.
