29 CFR 1926.451(a)(1): Each scaffold and/or scaffold component was not capable of supporting, without failure, its own weight and at least 4 times the maximum intended load applied or transmitted to it.  a) Front of Lots 6-9 - On or about November 14, 2014, an employee was working from a stack of unsupported plywood, which was not determined to meet the four time maximum load requirement, in a manner similar to a forklift supported scaffold at elevations up to 16 feet above grade.

29 CFR 1926.451(c)(2)(v): Fork-lifts were used to support scaffold platforms while the entire platform was not attached to the fork and the fork-lift was moved horizontally while the platform was occupied:  a) Front of Lots 6-9 - On or about November 14, 2014, an employee was working from a piece of unsupported plywood at elevations up to 24 feet above grade while the forklift was being moved.

29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:  a) Front of Lots 6-9 - On or about November 14, 2014, an employee was working from a stack of unsupported plywood, at elevations up to 16 feet above grade, and was not protected from falling to that lower level by any means of fall protection.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a) Lot 6-9, Worksite - On or about November 14, 2013, employees were conducting sheeting work, no less than 24 feet above a lower level, were not protected by guardrail systems, safety net system, or personal fall arrest system.  b) Lot 6-9, Worksite - On or about November, 14, 2013, employee were working within the framework of a residential home project with multiple window openings that were not protected by the equivalent of a standard guardrail.
