29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:    a. Entire facility - On or about November 15, 2013, the employer had not implemented an adequate energy control program for employees who perform servicing and maintenance on such equipment including but not limited to, air handlers, boilers, fans washer/dryer machines.  Employees were thereby exposed to the hazards associated with the unexpected energization of equipment.

Facility - On or about November 15, 2013, employees required to perform voltage testing (110- 480V) on such equipment but not limited to outlets, ballasts, sump pump motors and air handlers were not provided with training in the safety related work practices emphasized in 29 CFR 1910.331 - 1910.335.  The employees were thereby exposed to hazards of electrical shock and arc flash/blasts.

Facility - On or about November 15, 2013, employees were required to perform voltage testing (110- 480V) on such equipment but not limited to outlets, ballasts, sump pump motors and air handlers. The employees were not provided with electrical protective equipment including but not limited to fire retardant clothing, hard hat, face shield, and adequate voltage rated gloves.  The employees were thereby exposed to hazards of electrical shock and arc flash/blasts.
