29 CFR 1910.1030(c)(1)(iv):  The Exposure Control Plan was not reviewed and updated when necessary to reflect new or modified tasks and procedures which affect occupational exposure:      (a) On or about December 18, 2013, at the above located workplace, the employer did not ensure the Exposure Control Program was reviewed and updated wherever necessary to reflect new or modified tasks and procedures which affected occupational exposure. Sections of the plan that were not reviewed and updated when necessary to reflect new or modified tasks and procedures which affect occupational exposure included:     i) The Hepatitis B Vaccination Section, which did not address employees in Category II,  ii) The Transmission-Based Precautions, Waste Disposal Section which did not address dry forms of regulated waste,  iii) The Methods of Compliance Section, referred to Departmental Policies which did not exist at the time of the inspection,  iv) The Methods of Compliance Section, referred to a Section 6, which did not exist at the time of the inspection,  v) The Hepatitis B Vaccination and Post-exposure Evaluation and Follow-up Section, referred to a Section 3, which did not exist at the time of the inspection.

29 CFR 1910.1030(f)(2)(i):  Hepatitis B vaccination was not made available after the employee had received the training required in 29 CFR 1910.1030(g)(2)(vii)(I) or within 10 working days of initial assignment to employees who had occupational exposure to blood or other potentially infectious materials:    a) During the period of approximately July 18, 2013 and December 18, 2013, at the above located workplace, new employees having occupational exposure to blood or other potentially infectious materials were not effectively trained in the facility's bloodborne pathogen exposure control program and then offered the HBV series within the first ten working days of initial assignment.

29 CFR 1910.1030(f)(2)(iv): The employer did not assure that employees who declined the hepatitis B vaccination offered by the employer signed the statement in Appendix A.    a) During the period of approximately July 18, 2013 and December 18, 2013, at the above located workplace, new employees having occupational exposure to blood and/or other potentially infectious materials declined the hepatitis B vaccination series. The employer did not have the employees sign a declinations form (as stated in Appendix A).
