29 CFR 1926.200(g)(2): Traffic control signs or devices used for protection of construction workers did not conform to Part VI of the Manual of Uniform Traffic Control Devices: a) On or about November 19, 2013, and at times prior to that date , employees were crossing the live lane of traffic, were not effectively protected from struck-by-hazards. The employees were crossing the live lane of traffic without utilizing the flagger to stop traffic or other means of traffic control.
