29 CFR 1926.1060(a): The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would enable each employee to recognize hazards related to ladders and stairways and train each employee in the procedures to be followed to minimize these hazards: a) Front Entrance, Unit 5 - A ladder training program was not developed or implemented as evidenced by an employee using a portable metal extension ladder to reach above a canopy and the ladder was resting on a sloped unlevel surface, not secured while on a slippery concrete surface and the pitch of the ladder was at a 45 degree angle, on or about 11/20/2013. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
