29 CFR 1910.23(d)(1)(iii): Stairway(s) less than 44 inches wide having both sides open, did not have one stair railing on each side:     a.    On or about 11/19/2013, Boynes Trucking System Inc. - #14 Lindberg Bay-, St. Thomas, VI- Supply Storage area:  Employees were exposed to fall hazards while working over a stairway that was not equipped with one standard stair railing on each side.    NOTE:  Documentation of abatement must be submitted for this citation item as required by 29 CFR 1903.19(d).

29 CFR 1910.178(l)(6):  The employer did not certify that each operator had been trained and evaluated as required by this paragraph (l):    a.  On or about 11/19/2013, Boynes Trucking System Inc. - #14 Lindberg Bay-, St. Thomas, VI-  yard area:  Power industrial trucks were used on a daily basis to transport cargo materials & equipment and during loading/unloading operations.  The employer did not certify that each operator had been trained and evaluated.    Note:  Certification and documentation of abatement must be submitted for this citation item as required by 29 CFR 1903.19(c) and (d), respectively.

29 CFR 1910.253(b)(4)(iii):  Oxygen cylinders in storage were not separated from fuel-gas cylinders or combustible materials (especially oil or grease), a minimum distance of 20 feet (6.1 m) or by a noncombustible barrier at least 5 feet (1.5 m) high having a fire-resistance rating of at least one-half hour:    a)  On or about 11/19/2013, Boynes Trucking System Inc. - #14 Lindberg Bay-, St. Thomas, VI- mechanics shop area: Employees worked and walked by acetylene & oxygen gas cylinders which were stored together, left standing near a truck and were not separated .    NOTE:    Certification of abatement is not required for this citation item as abatement was documented by the compliance safety and health officer within 24 hours of the inspection.
