29 CFR 1910.178(l)(3)(ii)(A): Powered industrial truck operators did not receive initial training on surface conditions where the vehicle will be operated:      a) On and at times prior to October 31, 2013 forklift and clamp truck operators were not trained on the minimal acceptable overlap of the dock plate into a trailer bed.       b)  On and at times prior to October 31, 2013 forklift and clamp truck operators were not trained to verify for themselves that trailers had been secured properly from movement in a loading dock. It was a common practice for operators to accept the illumination of the green light next to the loading dock as a go ahead to enter the trailer with their vehicle without verifying that the trailer had been secured properly.       Abatement Certification Required.
