29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): (a) Machine shop area - On November 22, 2013 an at times prior to employees were exposed to a rotating chuck and tool on the Monarch Machine Co. metal lathe operating at approximately 10 revolutions per minute while drilling hole on the part. The lathe was not provided with a chuck guard and chip and coolant splash shields as required by the American National Standards Institute B 11.6-2001 Paragraph 6.21.
