29 CFR  1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are:  barrier guards, two-hand tripping devices, electronic safety devices, etc.  a)  155 Lister Lane, Creola, AL;  On or about November 16, 2022, and at times prior thereto, the employer exposed employee(s) to struck-by and caught-in hazards in that employees were allowed to operate the King Rotary Table machine which does not have a barrier that protects employees from flying metal/steel chips and fragmentation (i.e. shrapnel) from breaking tool bits, or guarding to prevent rotating table/part caught-in injury(ies).  b)  155 Lister Lane, Creola, AL;  On or about November 16, 2022, and at times prior thereto, the employer exposed employee(s) to struck-by hazards in that employees were allowed to operate the Summit Horizontal Mill machine which does not have a barrier that protects employees from flying metal/steel chips and fragmentation (i.e. shrapnel) from breaking tool bits.  c)  155 Lister Lane, Creola, AL;  On or about November 16, 2022, and at times prior thereto, employee(s) engaged in maintenance activities on the Mazak 510 Vertical Mill machine, were exposed to amputation, caught-in, and struck by hazards in that employee(s) was/were allowed to perform service and/or maintenance on equipment without an alternative safety measure or effective retraining after a change in a process.  d)  155 Lister Lane, Creola, AL;  On or about November 16, 2022, and at times prior thereto, the employer exposed employee(s) to pinched-between hazards in that employees were allowed to operate the Cincinnati Rotary Table machine with a damaged/dented-in tubular guard-rail whereby the dented tubular guard-rail creates an in-going pinch point between the rotating table and the tubular guard, when the table is rotating.  e)  155 Lister Lane, Creola, AL;  On or about November 16, 2022, and at times prior thereto, the employer exposed employee(s) to struck-by hazards in that employees were allowed to operate the EnerPac Vertical Hydraulic Arbor Press machine which does not have a barrier/barriers to protect employees from flying parts, blocks, pins, debris and/or pressurized fragmentation (i.e. shrapnel) occurring when parts may break during extreme hydraulic pressurization.

29 CFR  1910.253(b)(2)(ii): Inside of buildings, cylinders shall be stored in a well-protected, well-ventilated, dry location, at least 20 (6.1 m) feet from highly combustible materials such as oil or excelsior. Cylinders should be stored in definitely assigned places away from elevators, stairs, or gangways. Assigned storage spaces shall be located where cylinders will not be knocked over or damaged by passing or falling objects, or subject to tampering by unauthorized persons.   a)  155 Lister Lane, Creola, AL; On or about November 16, 2022, and at times prior thereto, the employer exposed the employees to struck-by hazards in that oxygen and propylene/petroleum cylinders were stored in an unprotected corner adjacent to the Shop and Fabrication Bays, and were stored (i.e. no barriers, chains/cables, obstructions or bump-guards) where they could easily be struck, knocked over or damaged.
