29 CFR 1910.219(c)(2)(i): All exposed part(s) of horizontal shafting seven (7) feet or less from floor or working platform were not protected by stationary casing(s) enclosing shafting completely or by trough(s) enclosing sides and top or sides and bottom of shafting:        a)  On or about November 26, 2013, and at times prior thereto employees exposed to a caught-in hazard when rotating shafts in the operators area on the Bergandi 2MT chain link fence weaver machine number three of five in the main production area were not adequately guarded.        b) On or about November 26, 2013, and at times prior thereto employees exposed to a caught-in hazard when rotating shafts in the operators area on the Bergandi 2MT chain link fence weaver machine number four of five in the main production area were not adequately guarded.        c) On or about November 26, 2013, and at times prior thereto employees exposed to a caught-in hazard when rotating shafts in the operators area on the Bergandi 2MT chain link fence weaver machine number five of five in the main production area were not adequately guarded.

29 CFR 1910.219(f)(3): Sprocket wheels and chains which were seven -7 feet or less above floors or platforms were not enclosed:       a)  On or about November 26, 2013, and at times prior thereto employees exposed to a caught-in hazard when gears and sprockets on the second in line Bergandi 2MT chain link fence weaver machine from the office near the rear of the main production area were not adequately guarded.           b)  On or about November 26, 2013, and at times prior thereto employees exposed to a caught-in hazard when gears and sprockets on the thrid in line Bergandi 2MT chain link fence weaver machine from the office near the rear of the main production area were not adequately guarded.        c) On or about November 26, 2013, and at times prior thereto employees exposed to a caught-in hazard when gears and sprockets on the fourth in line Bergandi 2MT chain link fence weaver machine from the office near the rear of the main production area were not adequately guarded.        d) On or about November 26, 2013, and at times prior thereto employees exposed to a caught-in hazard when gears and sprockets on the fifth in line Bergandi 2MT chain link fence weaver machine from the office near the rear of the main production area were not adequately guarded.

29 CFR 1910.303(g)(2)(i): Except as elsewhere required or permitted by Subpart S of Part 1910, live parts of electric equipment operating  at 50 volts or more were not guarded against accidental contact by use of approved cabinets or other forms of approved enclosures or by any of the means identified in paragraphs (A), (B), (C), and (D) of 29 CFR 1910.303(g)(2)(i):        a)  On or about November 26, 2013, and at times prior thereto employees exposed to an electrical shock hazard when the control box on the second Bergandi 2MT chain link fence weaver machine in line from the office was not adequately covered, exposing employees to an electrical shock hazard of 120 volts.        b) On or about November 26, 2013, and at times prior thereto employees exposed to an electrical shock hazard when the control box on the third Bergandi 2MT chain link fence weaver machine in line from the office was not adequately covered, exposing employees to an electrical shock hazard of 120 volts.        c) On or about November 26, 2013, and at times prior thereto employees exposed to an electrical shock hazard when the control box on the fourth Bergandi 2MT chain link fence weaver machine in line from the office was not adequately covered, exposing employees to an electrical shock hazard of 120 volts.        d) On or about November 26, 2013, and at times prior thereto employees exposed to an electrical shock hazard when the control box on the fifth Bergandi 2MT chain link fence weaver machine in line from the office was not adequately covered, exposing employees to an electrical shock hazard of 120 volts.
