29 CFR 1910.22(c): Covers and/or guardrails were not provided to protect personnel from the hazards of open pits, tanks, vats, ditches, etc.: (a) On or about 11/21/13 - at the Transfer Press area, employees are exposed to fall and tripping hazards when the bolster scrap openings are not covered by a die or other piece of equipment and employees stand and walk on the bolster to conduct work such as but not limited to setting or removing dies and maintaining the bolster surface.

29 CFR 1910.179(l)(2)(ii): Cranes were operated before all guards had been removed, safety devices reactivated, and maintenance equipment was removed after adjustments and repairs had been made: (a) On or about 11/18/13 - at the Transfer Press area, the East crane was released for operation and operated before safety devices such as but not limited to the Magnetek Laser Guard (laser anti-collision sytem) were reactivated after a repair had been made.
