29 CFR 1926.20(b)(2): The employer did not initiate and maintain a safety program which provides for frequent and regular inspections of jobsites:    JOB SITE: 811 Center Street, Fulton, MO 65251, on or about 11/26/2013, at this worker site, the employer failed to provide a competent person to conduct frequent and regular inspections of the jobsite to determine the type PPE required to use while operating power tools, Fall Protection Requirements & training for a low slope roof, and the proper setup of an extension ladder.  The workers were exposed to struck-by and fall hazard.

The workers were exposed to a struck-by hazard.

The workers were exposed to a fall hazard.    The HERZING ROOFING, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(10), which was contained in OSHA inspection number 315541615, citation number 1, item number 1, issued on 1/10/2012, and was affirmed as a final order on or about 1/31/2012 with respect to a workplace located at 1527 Union Avenue, Moberly, MO 65270.

29 CFR 1926.503(a)(2): The employer did not assure that each employee exposed to fall hazards was trained by a competent person qualified in the areas specified in 29 CFR 1926.503 (a)(2)(i) through (viii):    JOB SITE: 811 Center Street, Fulton, MO 65251, on or about 11/26/2013, the employer failed to trained his workers the fall hazards associated with this work site. Workers were allowed to actively work on a 3:12 pitch roof, approximately 13 feet 1 inch eave to ground level without any form of fall protection neither (conventional nor alternative), and were not aware or familiar with the fall protection requirements. The workers were exposed to a fall hazard.    The HERZING ROOFING, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.503(a)(12)(I), which was contained in OSHA inspection number 315625772, citation number 1, item number 1a, issued on 8/30/2011, and was affirmed as a final order on or about 9/19/2011, with respect to a workplace located at 336 Northeast Road, Universal City, TX 78148.
