29 CFR 1910.213(h)(1): The sides of the lower exposed portion of the blade of radial saw(s) were not guarded to the full diameter of the blade by a device that automatically adjusted itself to the thickness of the stock and remained in contact with stock being cut: a)Mill Area-The Delta Radial Arm Saw blade guard did not adjust to the stock, but was fixed above the lower portion of the blade, exposing employees to amputation hazards on or about 2/05/14.

29 CFR 1910.243(c)(1): Abrasive wheel(s) were used on portable grinder(s) which were not provided with safety guard(s) meeting the requirements specified in 29 CFR 1910.243(c)(1) through (c)(4)(a): On or about November 22, 2013, in the Bennett Building Systems shop fabrication area: a) Employees performing metal and wood building manufacturing work were exposed to amputation hazards when they used Dewalt DC411 4.5-inch grinders without the guards installed.

29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toeboards: On or about November 22, 2013, at the Bennett Building Systems fabrication shop: Employees required to work from the top of the buildings being fabricated were exposed to fall hazards of greater than approximately 9.1-feet when they worked without fall protection including but not limited to guardrails. Or in the alternative: 29 CFR 1910.28(a)(1): Scaffolds were not furnished or erected in accordance with this standard for persons engaged in work that could not be done safely from the ground or from solid construction: Employees required to work from the top of the buildings being fabricated were exposed to fall hazards of greater than approximately 9.1-feet when they worked from the roofs of the buildings without scaffolding erected around the entire building under construction.
