29 CFR 1910.95(d)(1): When information indicated that any employee's exposure equaled or exceed the 8-hour time-weighted average of 85 decibels, the employer did not develop and implement a monitoring program:   Fabrication Department-In the manufacturing of sheds, employees in the production area were exposed to noise levels between 86-98 dBA.  Noise monitoring to determine the levels of noise employees were exposed to had not been conducted, on or about 01/14/14.    No abatement certification or documentation is required for this item.

29 CFR 1910.95(g)(1): The employer did not establish and maintain an audiometric testing program as provided by 29 CFR 1910.95(g) by making audiometric testing available to all employees whose exposures equal or exceed an 8-hour time-weighted average of 85 decibels:   a) In the fabrication area, an employee working on wall build was exposed to noise at a time-weighted average (TWA) of 97.6 dBA.  An audiometric testing program was not maintained for the temporary employees.  The sample was obtained from a 443 minute period.  Zero exposure was assumed for the 37 minute unsampled period, on or about 1/14/14.   b) In the fabrication area, an employee working on bottom metal was exposed to noise at a time-weighted average (TWA) of 95.2 dBA.  An audiometric testing program was not maintained for the employees.  The sample was obtained from a 445 minute period.  Zero exposure was assumed for the 35 minute unsampled period, on or about 1/14/14.   c) In the fabrication area, an employee working on wall build was exposed to noise at a time-weighted average (TWA) of 94.05 dBA.  An audiometric testing program was not maintained for the employees.  The sample was obtained from a 414 minute period.  Zero exposure was assumed for the 66 minute unsampled period, on or about 1/14/14.   d) In the fabrication area, an employee working on top metal was exposed to noise at a time-weighted average (TWA) of 86.3 dBA.  An audiometric testing program was not maintained for the employees.  The sample was obtained from a 425 minute period.  Zero exposure was assumed for the 55 minute unsampled period, on or about 1/14/14.   e) In the welding area, an employee welding was exposed to noise at a time-weighted average (TWA) of 88 dBA.  An audiometric testing program was not maintained for the temporary employees.  The sample was obtained from a 442 minute period.  Zero exposure was assumed for the 38 minute unsampled period, on or about 2/05/14.

The employer did not utilize protective welding screens and/or shields in areas adjacent to where welding work was being performed, on or about December 17, 2013.

The employer failed to ensure a written hazard communication program was developed and implemented prior to chemical use, on or about December 17, 2013.   In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

The employer failed to ensure all employees were trained on the new label elements (pictograms) and SDS format, on or about December 17, 2013.    In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
