29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:     Location:  898 Teson Road, Hazelwood, MO  63042    At the job site, workers were exposed to a fall hazard as they were not protected by any type of fall protection system while they actively performed roofing work on the low slope residential roof.  The eave to ground height measured approximately 9 feet, 6 inches at the side of the structure where workers were shingling.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  Location:  898 Teson Road, Hazelwood, MO  63042  The employer failed to provide a training program and to train the workers to recognize the hazards of falling and the procedures to minimize the fall hazards.  The employer allowed the workers to actively shingle on a residential low slope roof, approximately 9 feet, 6 inches above ground without any form of fall protection.

29 CFR 1926.1053(b)(1):  Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:      Location:  898 Teson Road, Hazelwood, MO  63042    Two employees gained access and egress by utilizing a portable ladder which did not extend at least 36" above the upper landing (roof).  The side rails measured approximately 24" above the landing.  This improperly positioned ladder was located at the roof eave along the North side of the residential structure that was being re-roofed.  The fall hazard at ladder landing measured approximately 9 feet.
