29 CFR 1926.1053(b)(1):  Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:      Location:  898 Teson Road, Hazelwood, MO  63042    Two employees gained access and egress by utilizing a portable ladder which did not extend at least 36" above the upper landing (roof).  The side rails measured approximately 24" above the landing.  This improperly positioned ladder was located at the roof eave along the North side of the residential structure that was being re-roofed.  The fall hazard at ladder landing measured approximately 9 feet.
