The worker were exposed to struck-by hazards.

On or about 10/19/2021, employees from Cesar Avalos Roofing were installing roofing materials on a 6:12 sloped roof, approximately 9 feet from ground level, of a residential building located at lot 452 Austin Dr., Herculaneum, MO 63048. The employees were not protected by guardrails systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measures. The workers were exposed to a fall hazard.

On or about 10/19/2021, an employee from Cesar Avalos Roofing were installing roofing materials on a roof of a new residential building located at lot 452 Austin Dr, Herculaneum, MO 63048. The owner and one employee were using 2 extension ladders to access the roof. Both ladders was not extended three feet beyond the landing surface or edge of the roof.
