29 CFR 1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system: On or about 11/20/13 at a residential construction site located at the intersection of Laurel Park Lane and Selby Court in Clarence, New York; employees installing shingles on a two story house were using a ladder jack scaffold with a platform height of approximately 11 feet. Employees on the scaffold did not have a personal fall arrest system. NO ABATEMENT CERTIFICATION REQUIRED

The roof had a 12 in 12 pitch and there were no fall protection systems in use. NO ABATEMENT CERTIFICATION REQUIRED
