29 CFR 1928.57(a)(6): At the time of initial assignment and at least annually thereafter, the employer did not instruct every employee in the safe operation and servicing of all covered equipment with which he was or would be involved: a. On or about November 15, 2013, an employee operating a tractor was not adequately trained in that he did not make sure everyone was clear of the machinery before starting the engine, engaging power, and operating the machine, exposing employees to a crush hazard.
