29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:  a) On or about 5/7/14 - at Building A, 115 Greensboro Ave., Tuscaloosa, AL, employees were not protected from falling about 9-feet at an unprotected side or edge where the job-made ladder was located between the second and third floors near the frame marked unit 211 type C2A.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501(b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:  a) On or about 5/7/14 - at Building A, 115 Greensboro Ave., Tuscaloosa, AL, employees were not protected from falling about 9-feet when installing floor joists and working at the leading edge.

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder:  (a) On or about 5/7/2014 - at Building A, 115 Greensboro Ave., Tuscaloosa, AL, the job made ladder between the second and third floors near wall identified as unit 211 type C2A did not extend at least 3 feet above the upper landing surface and a grasping device was not provided.
