Extension cords were used to wire permanent fixtures involving receptacles mounted in the benches.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief is provided that will prevent pull from being directly transmitted to joints or terminal screws.   a) On or about December 02, 2013 and at times prior the employer did not ensure employees were protected from electrical shock in the wood shop due to not providing strain relief on electrical equipment and electrical cords as follows:  1)  Hanging extension cords attached to the ceiling throughout the wood shop were not provided with strain relief to prevent pull from being directly transmitted to joints or terminal screws.   2)  An electrical cord connected to the disk sander motor had the jacket pulled back and frayed.
